As of today, you are eligible to receive the COVID-19 vaccine as part of NY State’s phase 1b immunization plan. School districts should be notifying all staff of the necessary steps to be taken by any employee who is interested in receiving the vaccine.
However, in case you have not yet received any information from your respective school district and are interested in receiving the vaccine, you can fill out the attached NYSDOH form to arrange an appointment. It is uncertain as to how soon that appointment can be scheduled, but members who are interested in receiving the vaccine are encouraged to try and schedule an appointment promptly.
