Dear CAS Administrator,
This email is just to inform you that all active dues-paying members of the Council of Administrators and Supervisors (CAS) are covered by a group Life Insurance Policy insured by CIGNA Life Insurance Company of New York (CLICNY). In addition to life insurance coverage, this policy includes Accidental Death and Dismemberment (AD&D) insurance coverage, a Terminal Illness Benefit, and a Limited Seatbelt Benefit. This insurance coverage is provided to all active dues-paying CAS members at no additional cost. Here are some highlights of the CIGNA insurance coverage now available:
- Basic Life Insurance Coverage – $10,000 in life insurance benefits for you.
- AD&D Coverage – pays benefits for covered accidents, including a Limited Seatbelt Benefit.
Unfortunately, CAS members have passed away suddenly without designating a beneficiary for their CAS Life Insurance policy. If you wish to designate one or more beneficiaries, you must complete the Beneficiary Designation Form and return it to Andrew Buttelman, Lincoln Investment, 640 Johnson Avenue, Suite 204, Bohemia, NY 11716. If you do not choose a beneficiary, life insurance proceeds may be paid directly to any of the following, at the option of the insurer: (a) your spouse; (b) your child or children; (c) your parents; (d) your sisters or brothers; or (e) your estate. Please protect your family and take a few minutes to complete the Beneficiary Designation Form.
Here is your copy of your Certificate of Insurance. Please look at this document as it contains important details about the terms of the new policy, including exclusions, limitations, reduction of benefits, and terms under which the policy may be continued in force or discontinued.
